ECS Mandate Form — Notes
- Fill the complete form carefully
- Submit two copies to us and keep one copy with yourself
- Attach a cancelled cheque to verify MICR code
- Cheque must be a MICR cheque

Once we receive your form, we will submit it to our banker and donation installments will start deducting automatically.
Important Notes
- In case of ECS return, charges will be levied by your bank (each bank has its own return charge policy).
- Intimation will be given on your registered email and mobile 7 days before presenting the ECS form for each installment.
- If you do not want a deduction in any particular month, inform us at least 7 days before via:
- Ensure sufficient funds are available in your account on the date selected in the ECS Mandate Form.
- No charges will be levied from your account to process the mandate form.
Instructions to Fill the Form
- Donor Name
- Name in which the Tax Certificate / Donation receipt is required.
- Email / Phone / Address
- Where all communication (receipts, updates, intimations) can be sent.
- Donor ID
- If you have donated previously, your NG code / Donor ID is printed on your computer-generated receipt. If you don't remember, mention it on the back of the form — we'll locate it.
- Start Date
- Choose a date between 5th and 10th of any month. Must be later than 30 days from the dispatch date of your form.
- End Date
- The date on which the mandate expires. Must be later than: Start Date + (Installments × Frequency period).
- Signature
- Must match your signature as registered with your bank.

You want to donate ₹5,000 monthly for 2 years, today's date is 11/08/2018:
